Because it's a medical emergency when it happens. If it's not untwisted and blood supply restored in time, the ******** will die. Unless someone wants to go around being called One Nut Chuck for the rest of his life, he's gonna haul his ass to the hospital. Plus with the kind of pain I can only imagine would he associated with that, I'd be speeding to the hospital anyways to try and get the pain stopped.
