Get on a recumbent stationary bike and get your minutes in.
You don't have to stand to exercise (crunches, bike...)
water is great for low impact exercise, so if you don't have access to s bike try to get to a pool. also with this the Internet and physical therapists are your friend
There are great chair exercises videos on the Net. Also, Pilates.
are you 600 pounds?
Just start doing upper body exercises and free weights. Do what you can and push yourself without getting injured. You will find plenty of such exercises on youtube, something suitable for extreme over weight people should do fine as normally they don't give them leg/back heavy exercises.
"Please repeat back to me what I said, so I know you heard me." Is often a good sentence, OP. And if they refusing, asking for a patient advocate is a good idea.
