See a doctor! This can be a cardiac (heart) issue. The body action of bearing down is called the vagal response and people with heart conditions can sometimes have their blood pressure bottom out when they do it. Elvis died taking a poo, mostly likely due to an unknown heart issue and the action of bearing down really hard to take a poo. Sorry man but you need to see a doctor.
