Today, my dad called my by the family dog's name, and stared at me blankly as if struggling to remember my name when I was annoyed. FML

So you thought your best course of action in this scenario was to come on here? You should be sitting your daughter down, explaining to her why what she is doing is wrong, make her WRITE an apology and then present it to the bullied HERSELF. Take away anything with internet access(barring school related needs). Failure to act on this and punish her because of it will make you a failure of a parent.

I've actually been through this before, but my mom trips over more names than just one...I've been called the dead dog's name, the dead uncle's name, and two living uncle's names...I live with her...there's a reason I'm not attached to my name. good luck
