OP here. You're exactly right. We've lived together this entire time, and honestly, it doesn't come up very often. He's only met my family once, and the parents have different last names now. So it's pretty comical really. Thank you all for your thoughts! that was fun to share.

I agree forgetting names can be easy, i on occasion have to try and remember my own middle name since i never use it anywhere for anything.

If it never comes up or is never used, I can completely understand how it could be glossed over. Yeah it kind of stinks, but has he ever had to write out your full name? Does he mail out cards to your parents? Does he talk to your parents and called them "Mr & Mrs. ev_lyn_aw"? If everything has been first name basis your entire relationship, its entirely possible to happen. I worked as a company for 4 years, same people every day and I knew maybe 2 last names from 20 people (And thats because the names flowed together). Doesn't really matter if its a friend or a boyfriend. If your communication is the same, it can be missed or forgotten! If this is the worst of it, don't sweat it. Help remind him from time to time!
Yeah it's still totally weird. If you live together how come that he has never seen the doorbell or a letter or the rental agreement you both signed or anyone ever calling you by your last name. Don't you ever go on holidays together?
Putting him on the spot like that might have made his brain lock up, rendering him incapable of finding the answer even if he did know it (especially if it's the first time you've bothered to ask him). It's happened to me before. Try asking him again but don't just spring the question on him out of nowhere. My boyfriend of almost 4 years still has trouble remembering my middle name (Joe) when I ask. He has to think about it before it comes to him.
