1. It was most likely not a doctor, but a sonographer. There is a whole job built around ultrasound, and with the amount of ultrasounds done I don't know why it is so hard to call people by the right title.
2. It may not have been a pregnancy scan. You need a full bladder for a scan of your uterus etc. to help distinguish between structures.
3. There is such a thing as drinking too much water before a scan. If the bladder is too full it can compress other structures and make them harder to visualise.
