Sounds like you have very poor dental hygeine and have a diet high in sugars....

If you have cavities from poor hygiene then YDI, but sometime teeth grow in that way. My adult molars came in with crevices so deep that all 12 required fillings. This isn't necessarily Op's fault. Without more details there's no way to judge.
