Today, I went in for an appointment with my therapist. When she saw me, she gasped, "Are you okay? You look terrible!" I was great, actually, I just wasn't wearing any makeup. FML

Depending on what you're seeing her for, it could be that your mental and emotional health has been/is negatively effecting your physical health. Which, since it was the first time she's seen you without make up, it was the first time she's seen the toll your mental and emotional state has done to your body. If that's the case, most of the damage will go away over time as you stay in a better state of mind. Seriously, mental and emotional problems exact a huge toll on your body- I've had people pull me aside and essentially ask if I'm an abused child because my eyes look perpetually bruised from the severity of my sleep issues alone. As difficult as it might be, try not to overthink it.

Try not to dwell on it too much OP. Some people seem to be surprised when they see someone without their makeup on for the first time, since makeup can really change appearances pretty well, or enhance I should say. You don't need it to be beautiful though, you're beautiful for being you :)

Terrible therapist! Why did they make such a comment on physical appearance? I'd find someone else.

Heaven forbid that a mental and emotional health professional express concern for a patient's mental and emotional well being! They commented because they thought something very damaging to OP's emotional and mental health had happened, and that it was effecting their physical health. It's that simple. Telling OP to get a new therapist because the current one did their job is a bit ridiculous.
