You wouldn't know. And assuming you weren't a dick when you asked why she was saying no then it's stupid to go to your manager. If you have a good manager they will back you up.
If you happen to work in a place where the disease may be common among customers and you have worked there a while, then you should know what to look for. I have worked at two drugstores and have had my fair share of customers who have Parkinson's; it is pretty easy to tell purposeful shaking from their shaking. If that is not the case, then I suppose you could not have known.
wow, that's a huge assumption to make. What on earth makes you think the OP works in a place where customers with Parkinson's are common? I assumed he just worked in a regular department store. Maybe he worked in a fast food outlet, or a jewellery shop. I don't think there's any way to judge whether he should have known or not.

Ouch, sucks fo' you. My great-grandma had that terrible, but it never caused her to shake her head that way. Oh well, you didn't know! I understand her being a bit hurt for the misunderstanding, but no need in talking to the manager about that.
