Assumptions

Today, the doctor saw in my file that I was 39 and asked me straight away, “How many kids do you have?” Not IF I had kids, just how many. Sure, a woman's main purpose is to procreate. What else could she want from life? FML

Zero is a number. The question is perfectly acceptable without any of the other stuff you added to it.

Why ask 2 questions when 1 will get you the answer you need? How many kids do you have? Zero, one, two, etc. Do you have any kids? Yes. How many? Why do you assume the doctor sees women as only tools for procreation? Maybe he's simply streamlining the process. Plus, at that age, any pregnancies are deemed geriatric with a whole slew of potential complications.

My guesses are: 1) A well-stocked kitchen 2) Good birthing hips 3) Tasty recipes to please her man. Let me know if I win! I hope the prize is time travel to the 19th century.
