definitely this, could have written the whole thing with a forgiving tone, making it sound like you're not at fault but rather was in the wrong place at the wrong time

I didn't mean it in a bad way. I'm just wondering what happened to make your boss make you do such a ridiculous things, which by the way, was way out of line.

Why would your boss force you to write a warning meant for yourself in the first place? Is he just immensely lazy, or is this some sort of bizarre form of punishment? This really needs a follow up, OP.

I'm not sure on the rules, but I'm fairly certain that workplace harassment is frowned upon in most places.

If he was "shockingly incompetent" then I think it's a pretty good way to make sure he's not incompetent anymore.

You can usually type with one hand, so OP would have needed to crush both of them for this to be plausible.

Competence has as much to do with training as it does with personal responsibility. If an employee does something incompetent, it may very well be a case of poor/inadequate training. Having an employee write about their 'incompetence' would not fix this problem, were it the case.

Write up a praising report on how wonderful you are. Then, at the end, write "My boss wanted me to write up a negative report on myself but I'm not going to do it just to make him look good."

Hard to call this because it is an extreme measure. Are you a total zoidberg? Were you using the company's time on any social media? or does your boss have a bloated kanye west? is he harassing you for minimal reasons? this fml needs a follow up... me thinks
